This combination product pairs aspirin with two ingredients that calm the brain and nervous system: codeine (an opioid) and phenobarbital (a barbiturate). Both can already make you drowsy. Phellodendron contains berberine, and in animal studies berberine appeared to have its own mild sedative effect. So the worry here is simple: stacking phellodendron on top of these two could add up to more sleepiness, slower breathing, dizziness, or feeling foggy and uncoordinated.

Keep in mind this is a theoretical concern based on lab and animal data, not strong human studies, so the real-world risk is uncertain. Still, because codeine and phenobarbital are genuine CNS depressants, it is worth a quick chat with your pharmacist or doctor before you add phellodendron, especially if you ever feel unusually drowsy.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP2D6.
Phellodendron contains berberine. In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, phellodendron might also inhibit CYP2D6.

Here's the part that matters most in this combo product: the codeine. Codeine is a prodrug, which means your body has to switch it on using an enzyme called CYP2D6 to turn it into morphine, the part that actually relieves pain. Berberine (found in phellodendron) can slow that enzyme down. So in theory, phellodendron could mean less codeine gets activated, leaving you with weaker pain relief rather than more.

The aspirin and phenobarbital pieces aren't really affected by this CYP2D6 issue. I'll be honest though, this interaction is theoretical and based mostly on lab work, so it may not amount to much in real life. Still, it's worth a quick chat with your pharmacist or doctor before pairing them.

Phellodendron contains berberine. In vitro and in vivo research suggest that berberine can inhibit platelet aggregation. Theoretically, phellodendron might also inhibit platelet aggregation.

This combination pill contains aspirin, which is the piece that matters here. Aspirin already thins your blood by keeping platelets from clumping together. Phellodendron contains berberine, and in lab and animal studies berberine also seems to slow platelet activity. Stack the two and, in theory, you could bleed a bit more easily, think easy bruising, nosebleeds, bleeding gums, or a cut that takes longer to stop.

I want to be honest about the evidence: this is theoretical, based on test-tube and animal work, not human studies, so it's a caution rather than a red alert. Still, since aspirin is already a blood thinner, it's worth a quick chat with your pharmacist or doctor before adding phellodendron, especially if you notice any unusual bleeding.
