Here's the simple version: both of these slow down your brain and nervous system. Phenobarbital is a barbiturate, one of the strongest sedatives we have, and phenibut is also a powerful depressant. Stacking two calming agents like this can pile on their effects. So we worry about heavy drowsiness, confusion, slurred speech, slowed breathing, and in a bad case, breathing that becomes dangerously shallow.

To be fair, this concern is based on how these drugs work rather than solid studies, but with phenobarbital it's a real-world worry, especially at higher doses. Honestly, I'd steer clear of phenibut altogether and talk with your doctor or pharmacist about safer options. Please don't change your phenobarbital on your own.
