Budenoside falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Polypodium Leucotomos interacts with.

Theoretically, Polypodium leucotomos might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggests that Polypodium leucotomos can decrease the metabolism of midazolam by a mechanism possibly related to the inhibition of CYP3A4. However, a small study in humans suggests that taking Polypodium leucotomos with midazolam does not affect the metabolism of midazolam.

Here's the short version on pairing this fern extract with budesonide. Budesonide is a steroid that your liver clears using an enzyme called CYP3A4. The theory is that Polypodium leucotomos could slow that enzyme down a little, which would let budesonide build up and slightly raise the chance of typical steroid effects (like throat irritation, mouth thrush with the inhaler, or more steroid in your system over time).

Honestly, I wouldn't lose sleep over this one. The lab and animal hints didn't hold up well in people, and a human study with a similar drug showed no real change. The evidence here is weak and this is rated minor and unlikely. Still, it never hurts to mention the supplement to your pharmacist or doctor so they can keep an eye on things.
