Fosaprepitant falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Polypodium Leucotomos interacts with.

Theoretically, Polypodium leucotomos might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggests that Polypodium leucotomos can decrease the metabolism of midazolam by a mechanism possibly related to the inhibition of CYP3A4. However, a small study in humans suggests that taking Polypodium leucotomos with midazolam does not affect the metabolism of midazolam.

Here's the gist: Fosaprepitant (Emend Injection) is given in the hospital or clinic to help prevent nausea and vomiting from chemotherapy. It's processed by a liver enzyme called CYP3A4, and the idea on file is that Polypodium leucotomos might slow that enzyme down a bit. In theory, that could let fosaprepitant linger a little longer and bump up effects like tiredness, hiccups, headache, or mild stomach upset.

That said, I wouldn't lose sleep over this one. The evidence is weak and mostly from lab and animal studies, and a small human study with a similar drug (midazolam) showed no real change. Since fosaprepitant is given by a care team during chemo, just let your oncology pharmacist know you take this fern extract so they can confirm it's fine.
