Isavuconazonium Sulfate (IV) falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Polypodium Leucotomos interacts with.

Theoretically, Polypodium leucotomos might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggests that Polypodium leucotomos can decrease the metabolism of midazolam by a mechanism possibly related to the inhibition of CYP3A4. However, a small study in humans suggests that taking Polypodium leucotomos with midazolam does not affect the metabolism of midazolam.

Here's the short version: your antifungal, isavuconazonium (Cresemba), is cleared by a liver enzyme called CYP3A4. In lab and animal studies, this fern extract looked like it might slow that same enzyme down a little. On paper, that could let the drug build up slightly, which in theory might mean more of its usual side effects like nausea, headache, or changes in liver blood tests.

But I want to keep this in perspective. When researchers actually tested Polypodium leucotomos in people (using a similar CYP3A4 medication), it did not meaningfully change how the drug was handled. So this is mostly a theoretical concern, rated minor and unlikely. Still, since Cresemba treats serious infections, it's worth a quick mention to your doctor or pharmacist before you add the supplement.

What Is Isavuconazonium Used For?

Brand name: Cresemba

Cresemba (isavuconazonium sulfate) is used to treat two serious, life-threatening fungal infections: invasive aspergillosis and invasive mucormycosis. These infections can spread through the body and are most dangerous in people with weakened immune systems.

Cresemba for injection is approved for adults and children 1 year of age and older. Cresemba capsules are approved for adults and children 6 years of age and older who weigh at least 16 kilograms (about 35 pounds).
