Pomalidomide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Polypodium Leucotomos interacts with.

Theoretically, Polypodium leucotomos might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggests that Polypodium leucotomos can decrease the metabolism of midazolam by a mechanism possibly related to the inhibition of CYP3A4. However, a small study in humans suggests that taking Polypodium leucotomos with midazolam does not affect the metabolism of midazolam.

Here's the gist of it. Your body clears pomalidomide (Pomalyst) partly through a liver enzyme called CYP3A4. In the lab, Polypodium leucotomos looked like it might slow that same enzyme down, and in theory that could let pomalidomide build up a little, nudging up its known effects like low blood counts, fatigue, infections, or blood clots.

The good news: this is mostly a theoretical worry. When researchers actually tested the fern extract with a similar CYP3A4 drug in real people, they saw no meaningful change. So a real-world problem is unlikely. Still, pomalidomide is a serious cancer medicine with tight safety monitoring, so please run any new supplement past your oncologist or pharmacist before starting it.

About Pomalidomide

What Is Pomalidomide Used For?

Brand name: Pomalyst

Pomalidomide capsules are used in adults with multiple myeloma (a blood cancer affecting plasma cells) who have already tried at least two prior treatments — including lenalidomide and a proteasome inhibitor — and whose disease has continued to progress. It is given in combination with dexamethasone for this use.

Pomalidomide capsules are also used to treat Kaposi sarcoma (KS) — a cancer that can cause lesions on the skin and in internal organs — both in adults with AIDS-related KS after antiretroviral therapy has stopped working, and in adults with KS who are HIV-negative. The KS indication was approved based on response rate; ongoing studies are confirming long-term benefit.
