Tamoxifen Citrate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Polypodium Leucotomos interacts with.
In vitro and animal research suggests that Polypodium leucotomos can decrease the metabolism of midazolam by a mechanism possibly related to the inhibition of CYP3A4. However, a small study in humans suggests that taking Polypodium leucotomos with midazolam does not affect the metabolism of midazolam.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the gist for tamoxifen. Your body uses several liver enzymes, including CYP3A4, to handle this medicine. Lab and animal studies hint that Polypodium leucotomos might slow CYP3A4 a little, which in theory could let some tamoxifen-related compounds build up slightly. In practice, though, the human evidence is reassuring. When researchers tested this fern extract with a sensitive CYP3A4 drug (midazolam) in people, they didn't see a meaningful change.
So this is rated as a minor and unlikely interaction, mostly theoretical. If levels did shift upward, you might notice more typical tamoxifen effects like hot flashes, nausea, or fatigue. I wouldn't be alarmed, but since tamoxifen is an important cancer medicine, please check with your pharmacist or doctor before adding this supplement.

What Is Tamoxifen Used For?
Brand names: Nolvadex, Soltamox
Tamoxifen is used to treat metastatic breast cancer (breast cancer that has spread) in both women and men, and as adjuvant (after-surgery) therapy for early-stage breast cancer in women. It also reduces the occurrence of cancer in the opposite breast when used as adjuvant therapy.
In women with ductal carcinoma in situ (DCIS) — a very early, non-invasive form of breast cancer — tamoxifen is used after breast surgery and radiation to reduce the risk of developing invasive breast cancer. It is also approved to reduce the incidence of breast cancer in women who are at high risk but have not yet been diagnosed. Soltamox (oral solution) is specifically indicated for all of these uses in adult patients.
