CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Ramelteon falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Pomegranate interacts with.
Some animal and in vitro research shows that pomegranate juice inhibits intestinal, but not hepatic, CYP2C9 isoenzyme activity. However, clinical research shows that neither pomegranate juice nor pomegranate extract have a significant effect on CYP2C9 activity in humans.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the short version: pomegranate and Rozerem (ramelteon) is a pretty low-worry combo. Ramelteon is partly handled by a liver enzyme called CYP2C9. In theory, if pomegranate slowed that enzyme down, a bit more ramelteon could linger in your system, which might make you feel groggier, drowsier, or dizzy the next morning.
But the real-world evidence doesn't back that up. Lab and animal studies hinted pomegranate might block this enzyme in the gut, yet actual human studies showed pomegranate juice and extract had no meaningful effect on CYP2C9. So enjoying pomegranate as a food or juice is very unlikely to change how your sleep medicine works. If you're using concentrated extracts, just give your pharmacist a quick heads-up.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Ramelteon also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Pomegranate contains several polyphenols that have individually been shown to inhibit CYP3A4. However, there is contradictory evidence about the effect of whole pomegranate juice on CYP3A4 activity. In vitro, pomegranate juice significantly inhibits the CYP3A4 enzyme, with comparable inhibition to grapefruit juice. In an animal model, pomegranate juice inhibits CYP3A4 metabolism of carbamazepine and increases levels of carbamazepine by 1.5 times; however, in human volunteers, drinking a single glass of pomegranate juice 240 mL or taking 200 mL daily for 2 weeks does not significantly affect levels of the CYP3A4 substrate midazolam after oral or intravenous administration. Another study in healthy volunteers shows that consuming pomegranate juice 300 mL three times daily for three days also does not significantly affect levels of simvastatin, a CYP3A4 substrate This suggests that pomegranate is unlikely to significantly affect levels of CYP3A4 substrates in humans.

Here's the gist for Ramelteon (Rozerem): your body relies on a liver enzyme called CYP3A4 to break it down. Pomegranate has natural compounds that can slow that enzyme in a test tube, so in theory it could let a bit more ramelteon linger in your system. If that happened, you might notice it more strongly, think extra drowsiness, grogginess, or feeling foggy the next morning.
The reassuring part is that this is mostly a theoretical concern. When researchers gave people real pomegranate juice, it didn't meaningfully change levels of similar CYP3A4 drugs. Enjoying pomegranate as a food or juice is unlikely to cause trouble. If you're using a concentrated extract, just mention it to your pharmacist or doctor to be safe.
