CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Testosterone Enanthate
Testosterone Enanthate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Pomegranate interacts with.
Pomegranate contains several polyphenols that have individually been shown to inhibit CYP3A4. However, there is contradictory evidence about the effect of whole pomegranate juice on CYP3A4 activity. In vitro, pomegranate juice significantly inhibits the CYP3A4 enzyme, with comparable inhibition to grapefruit juice. In an animal model, pomegranate juice inhibits CYP3A4 metabolism of carbamazepine and increases levels of carbamazepine by 1.5 times; however, in human volunteers, drinking a single glass of pomegranate juice 240 mL or taking 200 mL daily for 2 weeks does not significantly affect levels of the CYP3A4 substrate midazolam after oral or intravenous administration. Another study in healthy volunteers shows that consuming pomegranate juice 300 mL three times daily for three days also does not significantly affect levels of simvastatin, a CYP3A4 substrate This suggests that pomegranate is unlikely to significantly affect levels of CYP3A4 substrates in humans.

Here's the good news on this one: there's very little to worry about. Testosterone enanthate is partly broken down by a liver enzyme called CYP3A4, and pomegranate can block that enzyme in a test tube. In theory, blocking it could let testosterone hang around a bit longer and slightly raise its levels. But when researchers actually tested pomegranate juice in real people, it didn't meaningfully change blood levels of other CYP3A4 drugs. So this stays a theoretical concern more than a real one.
Practically speaking, enjoying pomegranate or its juice alongside your testosterone shots is unlikely to cause problems like extra acne, mood swings, or higher red blood cell counts. If you plan on using concentrated pomegranate extracts regularly, just give your pharmacist or doctor a heads-up.

About Testosterone Injection
What Is Testosterone Injection Used For?
Brand name: Xyosted
Testosterone injection is primarily used in men whose bodies do not produce enough testosterone on their own — a condition called hypogonadism. This includes primary hypogonadism (a problem with the testicles themselves, such as from injury, infection, or surgery) and hypogonadotropic hypogonadism (caused by problems in the brain's hormone signaling from the hypothalamus or pituitary gland). It is also used to stimulate puberty in carefully selected adolescent males with clearly delayed puberty who have not responded to other support.
Testosterone Enanthate Injection is also approved for use in women with advanced, inoperable metastatic breast cancer (specifically skeletal metastases) who are one to five years past menopause, or in premenopausal women with hormone-responsive breast cancer who have had their ovaries removed. This use should be directed by an oncologist. Aveed (testosterone undecanoate) is specifically approved for male hypogonadism only.
