Acetaminophen, Caffeine, Pyrilamine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

If you take a quercetin supplement alongside Midol Max Strength (which combines acetaminophen, caffeine, and pyrilamine), here's the honest picture: quercetin may gently tug on a liver enzyme called CYP3A4 that helps process some medicines. The catch is that the lab and small human studies don't even agree on the direction. In some cases quercetin seemed to slow that enzyme down, and in one study it actually sped it up.

For this particular product, none of the three ingredients leans heavily on CYP3A4, so any real-world effect is likely small. This is mostly theoretical, based on test-tube and animal work. Food amounts of quercetin almost certainly won't matter. If you're taking high-dose supplements regularly, just run it by your pharmacist or doctor first.
