Alogliptin, Metformin falls under ANTIDIABETES DRUGS — the group Quercetin interacts with.

Theoretically, concomitant use of quercetin and antidiabetes drugs might increase the risk of hypoglycemia.

Clinical research suggests that a combination of quercetin, myricetin, and chlorogenic acid reduce levels of fasting glucose in patients with type 2 diabetes, including those already taking antidiabetes agents. The effect of quercetin alone is unknown.

Here's the gist with Kazano (which combines alogliptin and metformin) and quercetin. Both of those medicines work to bring your blood sugar down. Quercetin may have a mild blood-sugar-lowering effect of its own, so stacking it on top of your diabetes pills could, in theory, push your sugar a little lower than expected. That raises the chance of hypoglycemia, which can feel like shakiness, sweating, dizziness, a fast heartbeat, or sudden hunger.

I'll be honest, this is mostly theoretical and the human evidence is on the weaker side. The studies that showed a real effect used quercetin mixed with other compounds, not quercetin by itself. Food amounts, like apples or onions, are very unlikely to matter. If you take a concentrated supplement, keep an eye on your readings and check in with your pharmacist or doctor before adding it.
