Aspirin, Caffeine, Orphenadrine falls under ORGANIC ANION TRANSPORTER 3 (OAT3) SUBSTRATES — the group Quercetin interacts with.
In vitro research shows that quercetin is a strong non-competitive inhibitor of OAT3, with half-maximal inhibitory concentration (IC50) values as low as 0.75 mcM. So far, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist of it. Your kidneys use a little transporter called OAT3 to help flush certain drugs out of your body. The aspirin part of this combo product leans on that transporter to clear out. In lab dishes, quercetin blocks OAT3 fairly strongly, so in theory it could slow the removal of aspirin and let a bit more of it hang around. That might nudge up aspirin-type side effects like stomach upset, heartburn, or easier bruising and bleeding.
The important part: this has only been seen in test-tube studies, never reported in real people, so I wouldn't lose sleep over it. Food amounts of quercetin are very unlikely to matter. If you're considering high-dose quercetin supplements, just run it past your pharmacist or doctor first.

Here's the gist with this combo pill (aspirin, caffeine, and orphenadrine). Your kidneys use a little transporter called OAT1 to help shuttle certain drugs, including the aspirin part, out of your body into the urine. In lab dish studies, quercetin blocks that OAT1 transporter. In theory, that could slow the removal of an OAT1 drug and let a bit more of it linger in your system, which can mean slightly stronger effects and more side effects (for aspirin, think stomach upset or ringing in the ears at higher levels).
I want to be honest though: this is only seen in test tubes so far, never reported in real people, and food amounts of quercetin are very unlikely to matter. If you're using high-dose supplements, just run it by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
3 of 3Aspirin, Caffeine, Orphenadrine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

This combination pill mixes aspirin, caffeine, and orphenadrine (a muscle relaxant). The piece that touches on CYP3A4 here is orphenadrine, which your liver helps clear using that enzyme. Quercetin can lightly tug on CYP3A4, and honestly the lab data point in both directions, sometimes blocking it and sometimes speeding it up, so we can't say for sure which way things would shift.
If quercetin slowed orphenadrine's breakdown, you might feel more of its side effects, like drowsiness, dry mouth, blurry vision, or a fast heartbeat (and caffeine could add to that jittery feeling). But this is mostly test-tube and animal evidence, so the real-world risk is probably small, especially with everyday food amounts. High-dose supplements are where I'd be more cautious. Please run it by your pharmacist before combining them.
