Quercetin and Cefmetazole

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Quercetin interacts with whole categories of medication. Cefmetazole falls into 2 of those categories, so it can interact with Quercetin in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ORGANIC ANION TRANSPORTER 3 (OAT3) SUBSTRATES

1 of 2

Why this applies to Cefmetazole

Cefmetazole falls under ORGANIC ANION TRANSPORTER 3 (OAT3) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might increase the effects and adverse effects of OAT3 substrates.

In vitro research shows that quercetin is a strong non-competitive inhibitor of OAT3, with half-maximal inhibitory concentration (IC50) values as low as 0.75 mcM. So far, this interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the gist: Cefmetazole (Zefazone) is an injectable antibiotic your kidneys help clear out of your body. They use a little doorway called OAT3 to push the drug into your urine. In lab studies, quercetin can block that doorway, which in theory could let cefmetazole hang around in your bloodstream a bit longer than usual and raise the odds of side effects like stomach upset, diarrhea, or other reactions.

Now, the honest part: this is only seen in test-tube research, not in real people. So I wouldn't lose sleep over it, especially with quercetin from foods. If you're taking high-dose quercetin supplements while getting cefmetazole, just mention it to your pharmacist or doctor so they can keep an eye on things.

ORGANIC ANION TRANSPORTER 1 (OAT1) SUBSTRATES

2 of 2

Cefmetazole also falls under ORGANIC ANION TRANSPORTER 1 (OAT1) SUBSTRATES.

Theoretically, concomitant use might increase the effects and adverse effects of OAT1 substrates.

In vitro research shows that quercetin is a strong non-competitive inhibitor of OAT1, with half-maximal inhibitory concentration (IC50) values less than 10 mcM. So far, this interaction has not been reported in humans.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Cefmetazole is an injectable cephalosporin antibiotic that your kidneys clear out of your body, partly with the help of a transporter called OAT1. Quercetin can block that transporter, at least in lab and test tube studies. In theory, blocking it could slow how fast cefmetazole leaves your system, letting more of it build up. That might mean a bit more antibiotic effect, but also a higher chance of side effects like stomach upset, diarrhea, or other reactions.

Here is the reassuring part: this is theoretical only, based on cell studies, and has never actually been reported in people. Everyday food amounts of quercetin are very unlikely to matter. If you are getting cefmetazole and take high-dose quercetin supplements, just mention it to your pharmacist or doctor so they can keep an eye on things.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Cefmetazole page

“Two entries on this list for cefmetazole (Zefazone) deserve your attention before the other 41, and both threaten the kidneys. Siberian cocklebur and asarabacca (a plant often contaminated with aristolochic acid, a compound the FDA considers unsafe) can each damage kidney function, and combining them with this antibiotic may add to tha…”

Quercetin
  • Seasonal allergies
  • Antioxidant support
  • Heart and blood pressure health
  • Anti-inflammatory support
  • Immune support
  • Exercise recovery

Quercetin is a plant flavonoid with antioxidant and anti-inflammatory properties found in many common foods and sold as a supplement. While early research is interesting for allergies, blood pressure, and inflammation, strong human evidence is limited for most uses. It is generally well tolerated in food amounts, but talk to your pharmacist or doctor before taking high-dose supplements.
