Why this applies to Glipizide

Glipizide falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2C9 substrates.

A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.

Here's the gist with glipizide and quercetin. Your body clears glipizide largely through a liver enzyme called CYP2C9. Quercetin, especially in higher supplement doses, appears to slow that enzyme down. So in theory, glipizide could hang around longer and act more strongly than expected.

For a blood sugar pill, that means a higher chance of low blood sugar (hypoglycemia), with signs like shakiness, sweating, dizziness, confusion, or feeling unusually hungry. The evidence here is mostly theoretical and from a small study using a different drug, so this isn't a sure thing, and quercetin from foods is unlikely to be a problem. If you take concentrated quercetin supplements, mention it to your pharmacist or doctor and keep an eye on your blood sugar readings.

Theoretically, concomitant use of quercetin and antidiabetes drugs might increase the risk of hypoglycemia.

Clinical research suggests that a combination of quercetin, myricetin, and chlorogenic acid reduce levels of fasting glucose in patients with type 2 diabetes, including those already taking antidiabetes agents. The effect of quercetin alone is unknown.

Here's the practical picture: glipizide already pushes your pancreas to release more insulin, which lowers your blood sugar. Quercetin, especially in higher supplement doses, may nudge blood sugar down a little too. Stack the two together and there's a chance your sugar could dip lower than you want, which can show up as shakiness, sweating, dizziness, a racing heart, or feeling confused or hungry.

I want to keep this in perspective, though. The evidence here is on the weaker side, and the research that showed a sugar-lowering effect actually used quercetin mixed with other plant compounds, so quercetin on its own is less certain. Quercetin from normal foods like apples and onions is very unlikely to be a problem. If you're considering a concentrated supplement, check with your pharmacist or doctor first and keep an eye on your home glucose readings.
