Here's the simple version: losartan (Cozaar) already lowers your blood pressure, and quercetin may nudge it down a little too. Stack the two and the effects can add up, which is why this pairing is flagged. The concern isn't with how your body breaks losartan down, it's just two things pulling blood pressure in the same direction.

If your pressure drops too far, you might feel dizzy, lightheaded, tired, or a bit woozy when you stand up quickly. Honestly, the human evidence here is modest, and quercetin from everyday foods (apples, onions, berries) is unlikely to cause trouble. Concentrated supplement doses are where I'd pay closer attention. Before adding a quercetin supplement, check with your pharmacist or doctor so they can keep an eye on your numbers.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2C9 substrates.

A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.

Here's a wrinkle worth knowing about. Losartan is actually activated in your body by an enzyme called CYP2C9, which turns it into its stronger active form. Quercetin can slow that same enzyme down. So in theory, blocking CYP2C9 could mean less of losartan's active form gets made, which might make your blood pressure medicine a little less effective.

I'll be honest, this is mostly based on lab work and animal studies, not solid human trials, so don't lose sleep over it. The amount of quercetin in foods like onions and apples is very unlikely to matter. High-dose supplements are the real question mark. If you take quercetin pills, just let your pharmacist or doctor know so they can keep an eye on your blood pressure readings.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's something worth knowing about pairing quercetin with losartan (Cozaar). Losartan is partly handled by a liver enzyme called CYP3A4, which converts it into its more active form. In animal studies, quercetin slowed losartan's breakdown and raised its levels in the blood. If that happened in people, you could see a slightly stronger blood-pressure-lowering effect, which might show up as lightheadedness, dizziness when standing up, or feeling unusually tired.

That said, I want to keep this in perspective. The evidence here is mostly from lab and animal work, and one human study actually pointed the opposite direction, so the real-world impact is uncertain. Everyday food amounts of quercetin are very unlikely to matter. If you're considering high-dose supplements, just check with your pharmacist or doctor first.

Theoretically, concomitant use might increase the effects and adverse effects of losartan and decrease the effects of its active metabolite.

Animal research shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) while decreasing plasma levels of losartan's active metabolite. This metabolite, which is around 10-fold more potent than losartan, is the result of cytochrome P450 (CYP) 2C9- and CYP3A4-mediated transformation of losartan. Additionally, in vitro research shows that quercetin may inhibit P-glycoprotein-mediated efflux of losartan from the intestines, resulting in increased absorption of losartan. These results suggest that concomitant use of quercetin and losartan might increase systemic exposure to losartan while also decreasing plasma concentrations of losartan's active and more potent metabolite.

Here's an interesting wrinkle with losartan: a good chunk of its blood pressure benefit actually comes from a more potent active metabolite your body makes after enzymes (CYP2C9 and CYP3A4) convert the drug. So losartan acts a bit like a partial prodrug. Lab and animal studies suggest quercetin may slow that conversion, leaving more parent losartan around but less of the stronger metabolite. It might also help more losartan get absorbed from your gut.

What could that mean in practice? Possibly slightly weaker blood pressure control, or shifts that show up as dizziness or lightheadedness. But honestly, this is theoretical, based only on test-tube and animal data, and food amounts of quercetin are unlikely to matter. If you're taking a high-dose supplement, just mention it to your pharmacist or doctor so they can keep an eye on your numbers.

What Is Losartan Used For?

Brand name: Cozaar

Losartan tablets — available as branded Cozaar and several generic products — are used to treat high blood pressure (hypertension) in adults and children 6 years and older. Lowering blood pressure reduces the risk of strokes and heart attacks. Losartan tablets are also used to reduce the risk of stroke in people with high blood pressure who also have left ventricular hypertrophy (an enlarged left side of the heart). Note: evidence suggests this stroke-risk benefit may not apply to Black patients.

Losartan tablets are additionally approved to protect the kidneys in people with type 2 diabetes, high blood pressure, and signs of kidney damage (diabetic nephropathy). Arbli, the oral suspension form, shares these same approved uses.
