CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

rosiglitazone, glimepiride falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Quercetin interacts with.
A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.

Avandaryl combines two diabetes medicines, and both rosiglitazone and glimepiride are partly cleared by a liver enzyme called CYP2C9. Quercetin can slow that enzyme down. In theory, that means these drugs could hang around longer in your body and have a stronger effect than expected. With glimepiride especially, a stronger effect can push your blood sugar too low, so watch for shakiness, sweating, dizziness, confusion, or feeling unusually hungry.
Keep in mind this is mostly based on lab work and studies using other CYP2C9 drugs, not Avandaryl directly, so the real-world risk is uncertain. Food amounts of quercetin are unlikely to matter much, but concentrated supplements could. Before starting a high-dose quercetin product, check with your pharmacist or doctor and keep an eye on your blood sugar readings.

ANTIDIABETES DRUGS

Clinical research suggests that a combination of quercetin, myricetin, and chlorogenic acid reduce levels of fasting glucose in patients with type 2 diabetes, including those already taking antidiabetes agents. The effect of quercetin alone is unknown.

Avandaryl pairs two diabetes medicines: rosiglitazone, which helps your body use insulin better, and glimepiride, which nudges your pancreas to release more insulin. Quercetin may have its own mild blood-sugar-lowering effect, so stacking it on top of these two could push your glucose lower than expected. The bigger concern is glimepiride, since drugs that raise insulin can cause real low blood sugar (shakiness, sweating, dizziness, confusion, or feeling hungry).
Honestly, this is mostly theoretical. The human study that hinted at lower fasting glucose used quercetin combined with other compounds, so quercetin alone is uncertain. Food amounts (apples, onions, berries) are very unlikely to matter. If you want to try a concentrated supplement, check with your pharmacist first and keep an eye on your home glucose readings.

CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES

In vitro research shows that quercetin inhibits CYP2C8. Inhibition of paclitaxel (Taxol) metabolism via CYP2C8 has been reported in vitro. However, a small study in humans found no effect of quercetin on rosiglitazone (Avandia), which is also a CYP2C8 substrate.

In theory, that could let these drugs build up a bit, which might push your blood sugar lower than expected. With glimepiride especially, that could mean more low blood sugar (shakiness, sweating, dizziness, confusion), and rosiglitazone can already cause fluid buildup and weight gain.
Here's the reassuring part: this concern comes only from lab work, and a small human study actually found that quercetin did not change rosiglitazone levels. So real-world risk looks low. Food amounts almost certainly don't matter. If you're considering high-dose quercetin supplements, just run it by your pharmacist or doctor first.
