Sacubitril, Valsartan falls under ANTIHYPERTENSIVE DRUGS — the group Quercetin interacts with.

Theoretically, taking quercetin with antihypertensive drugs might increase the risk of hypotension.

Quercetin can modestly decrease blood pressure in people with mild hypertension. Theoretically, it might have additive blood pressure lowering effects when used with antihypertensive drugs.

Here's the simple version: Entresto (sacubitril/valsartan) already lowers your blood pressure as part of how it helps your heart. Quercetin, in higher supplement doses, may nudge blood pressure down a little too. Stack them together and those small effects can add up, which is the concern here.

If your pressure drops too far, you might feel dizzy, lightheaded, unusually tired, or like you could faint when standing up. Keep in mind this is mostly a theoretical, additive concern based on modest evidence, and the small amounts of quercetin you get from foods like apples and onions are very unlikely to cause trouble. It's the concentrated supplements that warrant a heads-up. Before adding quercetin, please check with your pharmacist or doctor so they can keep an eye on your numbers.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Sacubitril, Valsartan also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2C9 substrates.

A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.

Here's the gist: the valsartan part of Entresto is partly cleared by a liver enzyme called CYP2C9, and concentrated quercetin supplements can slow that enzyme down. In theory, that means a bit more valsartan could stick around in your body, which could nudge its blood-pressure-lowering effect higher. Realistically, that might show up as feeling dizzy, lightheaded, or unusually tired, especially when you stand up quickly.

I want to be honest about the evidence here, though. This is mostly theoretical, drawn from a small study with a different drug and some animal research, so it's a possible concern rather than a proven one. Food amounts of quercetin are very unlikely to matter. Before starting a high-dose supplement, check in with your pharmacist or doctor so they can keep an eye on your blood pressure.

About Valsartan and Sacubitril

What Is Valsartan and Sacubitril Used For?

Brand name: Entresto

Sacubitril and valsartan is used to reduce the risk of cardiovascular death and hospitalization for heart failure in adult patients with chronic heart failure. The benefit is most clearly seen in patients whose heart pumps with a lower-than-normal ejection fraction (the percentage of blood the heart pushes out with each beat).

It is also approved to treat symptomatic heart failure with reduced left ventricular function in children aged 1 year and older. In pediatric patients, it lowers a blood marker of heart stress (NT-proBNP) and is expected to improve heart outcomes.
