Why this applies to Ticagrelor

Ticagrelor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the simple version: Brilinta (ticagrelor) is a blood thinner that keeps your platelets from clumping, and your body clears it using an enzyme called CYP3A4. Quercetin, in the lab, can block that same enzyme. If it does that in your body, ticagrelor could build up a bit, which in theory means more of its effects, including a higher chance of bruising or bleeding (bleeding gums, nosebleeds, or harder-to-stop bleeding).

I'll be honest with you though: the evidence here is weak and mostly comes from lab and animal studies, and one human study even hinted quercetin might speed CYP3A4 up instead. So this is more theoretical than proven. Food amounts of quercetin are unlikely to matter, but before starting a high-dose supplement, please run it by your pharmacist or doctor.

What Is Ticagrelor Used For?

Brand name: Brilinta

Ticagrelor tablets are used to lower the risk of cardiovascular death, heart attack, and stroke in adults who have had an acute coronary syndrome (ACS — a sudden reduction of blood flow to the heart) or a previous heart attack. They also reduce the risk of stent thrombosis (a blood clot forming in a coronary stent) in people stented during ACS treatment.

Ticagrelor tablets are also used to reduce the risk of a first heart attack or stroke in people with coronary artery disease (CAD) who are at high risk — including those with type 2 diabetes. Brilinta is additionally indicated to reduce the risk of stroke in adults with a mild acute ischemic stroke or a high-risk transient ischemic attack (TIA, sometimes called a "mini-stroke").
