Treprostinil (Inh) falls under ANTIHYPERTENSIVE DRUGS — the group Quercetin interacts with.
Quercetin can modestly decrease blood pressure in people with mild hypertension. Theoretically, it might have additive blood pressure lowering effects when used with antihypertensive drugs.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version for inhaled treprostinil (Tyvaso). This medicine works by relaxing and widening blood vessels to ease the pressure in your lungs, but that same action can lower your overall blood pressure too. Quercetin, taken as a concentrated supplement, may nudge blood pressure down a little on its own. Put the two together and the effects could stack up.
If your pressure drops too far, you might feel lightheaded, dizzy, flushed, or headachy, especially when standing up quickly. Keep in mind this is a theoretical, mild concern based on limited evidence, and quercetin from everyday foods like onions or apples is very unlikely to matter. High-dose capsules are the real question. Before adding one, check in with your pharmacist or doctor.

CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES
2 of 2Treprostinil (Inh) also falls under CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES.
In vitro research shows that quercetin inhibits CYP2C8. Inhibition of paclitaxel (Taxol) metabolism via CYP2C8 has been reported in vitro. However, a small study in humans found no effect of quercetin on rosiglitazone (Avandia), which is also a CYP2C8 substrate.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body clears inhaled treprostinil partly through a liver enzyme called CYP2C8. In lab (test-tube) studies, quercetin can slow that enzyme down. If that slowdown happened in real life, treprostinil could hang around a bit longer and its effects could feel stronger. With this drug that usually means more of its typical side effects, like flushing, headache, throat or cough irritation, jaw pain, low blood pressure, or feeling lightheaded.
I want to be honest about how weak this is. It's only theoretical, based on lab data, and a small human study on a similar CYP2C8 drug showed no real effect. Everyday food amounts of quercetin are very unlikely to matter. Before starting a high-dose supplement, just check with your pharmacist or doctor first.

What Is Treprostinil Oral Inhalation Used For?
Brand name: Tyvaso
Treprostinil oral inhalation is used to treat two serious lung conditions that raise pressure in the blood vessels of the lungs. It helps improve how much physical activity you can do.
Tyvaso inhalation solution and Tyvaso DPI are both indicated to treat pulmonary arterial hypertension (PAH; WHO Group 1) and pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3). Clinical studies establishing effectiveness in PAH primarily included patients with moderate symptoms (NYHA Class III) whose PAH was idiopathic, heritable, or related to connective tissue disease. Studies in PH-ILD primarily included patients with idiopathic interstitial pneumonia, combined pulmonary fibrosis and emphysema, or connective tissue disease-related lung disease.
