Why this applies to Zileuton

Zileuton falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2C9 substrates.

A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.

Here's the gist for your asthma medication. Zileuton is partly cleared by a liver enzyme called CYP2C9, and quercetin can slow that enzyme down. In plain terms, that means zileuton may hang around in your body a little longer than usual, which could nudge its levels and side effects upward. Think things like nausea, stomach upset, headache, or, less commonly, signs of liver stress (zileuton already requires liver monitoring).

I want to be honest about the strength here: this is mostly theoretical and based on studies with other CYP2C9 drugs, not zileuton itself. Occasional quercetin in foods is very unlikely to matter. High-dose supplements taken daily are the real question, so check with your pharmacist or doctor before combining them.

Zileuton also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the gist with Zileuton (Zyflo) and quercetin. Zileuton is partly handled by a liver enzyme called CYP3A4, and quercetin can nudge that enzyme. The tricky part is that the research points both ways: in some studies high-dose quercetin slows CYP3A4 (which could let a drug build up), and in at least one study it actually sped it up a little. So we honestly can't say for sure which direction this would go for Zileuton.

The good news is this is mostly lab and theoretical data, not strong human evidence, so I wouldn't lose sleep over it. Food amounts of quercetin (apples, onions, tea) are very unlikely to matter. If you're thinking about a concentrated supplement, just run it by your pharmacist or doctor first.

About Zileuton

What Is Zileuton Used For?

Brand names: Zyflo, Zyflo CR

Zileuton is used for the ongoing prevention and treatment of asthma in adults and adolescents aged 12 and older. It is a maintenance medicine — meaning it is taken regularly to keep asthma under control, not to stop an asthma attack once it has started.

Zyflo (the tablet form) and zileuton extended-release tablets are both approved for this same purpose. Neither form should be used to treat a sudden asthma attack. However, both can be continued during a flare-up of asthma while you also use your rescue inhaler.
