Why this applies to Adagrasib

Adagrasib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rabdosia Rubescens interacts with.

In vitro research shows that oridonin, a constituent of Rabdosia rubescens, induces cytochrome P450 3A4 (CYP3A4) enzyme. Also, in an animal model, oridonin induces a gene homologous to CYP3A4. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking Rabdosia rubescens. Some drugs metabolized by CYP3A4 that might be affected are some benzodiazepines such as alprazolam (Xanax), diazepam (Valium), midazolam (Versed), triazolam (Halcion); calcium channel blockers such as amlodipine (Norvasc), diltiazem (Cardizem, Dilacor, others), felodipine (Plendil), verapamil (Calan, Verelan, others); cyclosporine (Neoral, Sandimmune); some HIV antivirals such as indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase); some HMG CoA reductase inhibitors such as atorvastatin (Lipitor), lovastatin (Mevacor); some macrolide antibiotics such as erythromycin and clarithromycin (Biaxin); quinidine; and many others.

Here's the gist for your cancer medicine Adagrasib (Krazati). Your body clears this drug using a liver enzyme called CYP3A4. Lab studies suggest a compound in Rabdosia rubescens, called oridonin, may speed up that enzyme. If it does, your system could break down adagrasib faster than it should, leaving less in your blood. For a targeted cancer therapy, weaker drug levels are a real concern because the medicine may not work as well against the tumor.

The reassuring part: this is based only on test-tube and animal research, and it hasn't been confirmed in people. Still, because adagrasib is too important to risk, please don't combine them without first checking with your oncologist or pharmacist.

About Adagrasib

What Is Adagrasib Used For?

Brand name: Krazati

Krazati (adagrasib) is used to treat adults with locally advanced or metastatic non-small cell lung cancer (NSCLC) that carries a specific genetic change called a KRAS G12C mutation. It is used as a single agent for NSCLC after at least one prior treatment, and patients must be confirmed to have this mutation by an FDA-approved test.

Krazati is also used in combination with another drug called cetuximab to treat adults with KRAS G12C-mutated locally advanced or metastatic colorectal cancer (CRC) who have already been treated with specific prior chemotherapy regimens. Both uses were approved under the FDA's accelerated approval program based on tumor response data, and continued approval may depend on results from confirmatory trials.
