Why this applies to Avacopan

Avacopan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rabdosia Rubescens interacts with.

In vitro research shows that oridonin, a constituent of Rabdosia rubescens, induces cytochrome P450 3A4 (CYP3A4) enzyme. Also, in an animal model, oridonin induces a gene homologous to CYP3A4. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking Rabdosia rubescens. Some drugs metabolized by CYP3A4 that might be affected are some benzodiazepines such as alprazolam (Xanax), diazepam (Valium), midazolam (Versed), triazolam (Halcion); calcium channel blockers such as amlodipine (Norvasc), diltiazem (Cardizem, Dilacor, others), felodipine (Plendil), verapamil (Calan, Verelan, others); cyclosporine (Neoral, Sandimmune); some HIV antivirals such as indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase); some HMG CoA reductase inhibitors such as atorvastatin (Lipitor), lovastatin (Mevacor); some macrolide antibiotics such as erythromycin and clarithromycin (Biaxin); quinidine; and many others.

Here's the gist for Avacopan (Tavneos). Your body relies heavily on an enzyme called CYP3A4 to process this drug. Lab studies suggest that oridonin, a compound in Rabdosia rubescens, may speed up that enzyme. If that happens in people, your body could clear Avacopan faster than intended, leaving less of it in your system.

Why does that matter? Avacopan is doing important work to calm the immune attack on your blood vessels. If its levels drop too low, the medication may not control your condition as well. The good news: this is based only on test-tube and animal research, and it has not been seen in real patients. Still, since Avacopan is a serious treatment, please run any supplement past your pharmacist or doctor first.

What Is Avacopan Used For?

Brand name: Tavneos

Tavneos (avacopan) is used to treat severe active ANCA-associated vasculitis in adults — specifically the forms known as granulomatosis with polyangiitis (GPA) and microscopic polyangiitis (MPA). These are serious autoimmune conditions in which the immune system attacks small blood vessels, causing damage to the kidneys, lungs, and other organs.

Tavneos is used as an add-on treatment alongside standard therapies, including glucocorticoids (steroid medicines). It is not a replacement for steroids — it works together with them.
