Why this applies to Maraviroc

Maraviroc falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rabdosia Rubescens interacts with.

In vitro research shows that oridonin, a constituent of Rabdosia rubescens, induces cytochrome P450 3A4 (CYP3A4) enzyme. Also, in an animal model, oridonin induces a gene homologous to CYP3A4. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking Rabdosia rubescens. Some drugs metabolized by CYP3A4 that might be affected are some benzodiazepines such as alprazolam (Xanax), diazepam (Valium), midazolam (Versed), triazolam (Halcion); calcium channel blockers such as amlodipine (Norvasc), diltiazem (Cardizem, Dilacor, others), felodipine (Plendil), verapamil (Calan, Verelan, others); cyclosporine (Neoral, Sandimmune); some HIV antivirals such as indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase); some HMG CoA reductase inhibitors such as atorvastatin (Lipitor), lovastatin (Mevacor); some macrolide antibiotics such as erythromycin and clarithromycin (Biaxin); quinidine; and many others.

Your body relies on a liver enzyme called CYP3A4 to break down maraviroc. Lab studies suggest that oridonin, a compound in Rabdosia rubescens, may speed that enzyme up. If that happened in people, maraviroc could clear out of your system faster, leaving less of it to do its job.

Why does that matter? Maraviroc helps keep HIV under control, and if its levels drop, the virus has more room to grow and possibly become resistant. That makes this worth taking seriously. The good news is this is only test-tube and animal evidence so far, never confirmed in humans. Still, please don't add this herb without first checking with your HIV provider or pharmacist.

About Maraviroc

What Is Maraviroc Used For?

Brand names: Celsentri, Selzentry

Maraviroc treats a specific type of HIV-1 infection called CCR5-tropic HIV-1. It is always used alongside other antiretroviral (anti-HIV) medicines — never alone. A special test called a tropism assay must be done before starting treatment to confirm you have the right type of HIV.

Selzentry (the brand-name oral solution and tablet) is approved for adults and pediatric patients weighing at least 2 kg. Generic maraviroc tablets are approved for adults and pediatric patients aged 2 years and older weighing at least 10 kg. Neither form is recommended for patients whose HIV is classified as dual/mixed-tropic or CXCR4-tropic.
