Remdesivir falls under P-GLYCOPROTEIN SUBSTRATES — the group Rabdosia Rubescens interacts with.

Rabdosia rubescens might increase levels of p-glycoprotein substrates. In vitro research shows that Rabdosia rubescens extract inhibits p-glycoprotein levels and activity. Drugs that might be affected include some chemotherapeutic agents (doxorubicin, etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), protease inhibitors (amprenavir, indinavir, nelfinavir, saquinavir), H2 antagonists (cimetidine, ranitidine), some calcium channel blockers (diltiazem, verapamil), digoxin, corticosteroids, erythromycin, cisapride (Propulsid), fexofenadine (Allegra), cyclosporine, loperamide (Imodium), quinidine, and others.

Here's the situation with Remdesivir (Veklury) and this herb. Your body uses a kind of cellular bouncer called P-glycoprotein to push certain drugs back out. Lab studies suggest Rabdosia rubescens may block that bouncer, which could let a little more remdesivir hang around than expected.

I want to keep this in perspective. Remdesivir is given by IV in a medical setting, and the only evidence here comes from test-tube and animal research, not real people. So this is mostly theoretical. If more drug did build up, the main concerns would be nausea or liver enzyme changes, which your care team already watches closely. Still, let your doctor or pharmacist know about any supplement before treatment, just to be safe.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Remdesivir also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that oridonin, a constituent of Rabdosia rubescens, induces cytochrome P450 3A4 (CYP3A4) enzyme. Also, in an animal model, oridonin induces a gene homologous to CYP3A4. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking Rabdosia rubescens. Some drugs metabolized by CYP3A4 that might be affected are some benzodiazepines such as alprazolam (Xanax), diazepam (Valium), midazolam (Versed), triazolam (Halcion); calcium channel blockers such as amlodipine (Norvasc), diltiazem (Cardizem, Dilacor, others), felodipine (Plendil), verapamil (Calan, Verelan, others); cyclosporine (Neoral, Sandimmune); some HIV antivirals such as indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase); some HMG CoA reductase inhibitors such as atorvastatin (Lipitor), lovastatin (Mevacor); some macrolide antibiotics such as erythromycin and clarithromycin (Biaxin); quinidine; and many others.

Here's the short version on mixing Rabdosia rubescens with remdesivir (Veklury). One compound in this herb, oridonin, appears to speed up a liver enzyme called CYP3A4 in lab and animal studies. Remdesivir is partly handled by that same enzyme, so in theory, revving it up could clear remdesivir from your body a bit faster and leave you with lower levels. For an antiviral you're relying on to fight COVID-19, weaker drug levels could mean less of the punch you want.

That said, this is only test-tube and animal evidence, and it has never been seen in real patients. Also, remdesivir is given by IV in a medical setting, so your care team is watching closely. Still, please mention this herb to your doctor or pharmacist before using it.

What Is Remdesivir Injection Used For?

Brand name: Veklury

Veklury (remdesivir) is given by IV infusion to treat COVID-19 caused by the SARS-CoV-2 virus. It is approved for adults and for children from birth through age 17 who weigh at least 1.5 kg (about 3.3 lbs).

Veklury can be used for patients who are hospitalized with COVID-19, or for patients who are not hospitalized but have mild-to-moderate COVID-19 and are at high risk of getting worse — including being hospitalized or dying from the disease.
