Reishi Mushroom and Phenprocoumon

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTICOAGULANT/ANTIPLATELET DRUGS

Why this applies to Phenprocoumon

Phenprocoumon falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Reishi Mushroom interacts with.

Theoretically, high doses of reishi mushroom might increase the risk of bleeding.
A dose of 1.5 grams daily of reishi mushroom does not seem to decrease platelet aggregation, but a higher dose of 3 grams daily does.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Phenprocoumon is a blood thinner, much like warfarin, that lowers your blood's ability to clot. Reishi mushroom is a separate concern here. At higher doses (around 3 grams a day), reishi may make your platelets a bit less sticky, so it can add to the thinning effect of phenprocoumon. Stacked together, that could nudge your bleeding risk up: think easy bruising, nosebleeds, bleeding gums, or blood in your urine or stool.

The good news is this is mostly theoretical, based on lab and animal work, and lower reishi doses (about 1.5 grams) did not seem to affect platelets. Since phenprocoumon is a narrow-margin drug, please loop in your pharmacist or doctor before adding reishi so they can keep an eye on your INR.

From our pharmacist’s overview on the Phenprocoumon page

“Phenprocoumon (Liquamar) is a blood thinner in the coumarin family, a close cousin of warfarin, and small shifts in its blood level can tip you toward a clot or a bleed, so I treat this list with genuine care. Almost everything on it pushes the drug's effect further, layering extra bleeding risk on blood that is already thinned.”

  • Fatigue and general well-being
  • Stress and sleep
  • Inflammation
  • Heart and cholesterol health

Reishi is a traditional Asian mushroom widely used to support the immune system and overall wellness. Human evidence for most of its claimed benefits is limited or low-quality, so it should not replace proven treatments. It is generally well tolerated for short-term use, but check with your pharmacist or doctor before starting, especially if you are pregnant, nursing, or take other medicines.
