Tadalafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Resveratrol interacts with.

Theoretically, resveratrol might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that resveratrol can inhibit the CYP3A4 enzyme. However, clinical research shows that taking resveratrol 3000 mg daily for 8 weeks does not necessitate dose adjustments to medications metabolized by CYP3A4.

Here's the gist for tadalafil (Cialis): your liver relies on an enzyme called CYP3A4 to clear it out of your system. Resveratrol, in test-tube studies, can slow that enzyme down a bit. In theory, that could let tadalafil hang around longer and build up, which might mean stronger effects like headache, flushing, a stuffy nose, back pain, or a bigger dip in blood pressure (more lightheadedness).

The good news is that this is mostly lab-based thinking. Actual human research using fairly high resveratrol doses didn't require any dose changes for CYP3A4 medications. So for most people this is a low-concern, theoretical interaction. Still, if you take other blood pressure meds or nitrates, check with your pharmacist or doctor first.

About Tadalafil

What Is Tadalafil Used For?

Brand names: Cialis, Tadliq

Tadalafil tablets (including Cialis and generic tadalafil) are used to treat erectile dysfunction (ED), the symptoms of benign prostatic hyperplasia (BPH — an enlarged prostate), or both conditions together. When used with the drug finasteride to start BPH treatment, this combination is recommended for up to 26 weeks.

Adcirca, Alyq (tablets), and Tadliq (oral suspension) are indicated specifically for pulmonary arterial hypertension (PAH — high blood pressure in the lungs), to improve the ability to exercise.
