Rhubarb + Mephenytoin
Mephenytoin brand name: Mesantoin.
Rhubarb has a moderate interaction with Mephenytoin. Theoretically, concomitant use of rhubarb with potentially hepatotoxic drugs might increase the risk of developing liver damage.

HEPATOTOXIC DRUGS
Why this applies to Mephenytoin
Mephenytoin falls under HEPATOTOXIC DRUGS — the group Rhubarb interacts with.
Some animal research suggests that anthraquinones in rhubarb might have hepatotoxic effects. Also, rhubarb use has been linked to at least 24 cases of liver injury, although details on the dose of rhubarb and duration of use in these cases is unclear.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with rhubarb root and mephenytoin (Mesantoin). This isn't about rhubarb changing how the drug works in your body. Instead, both can be tough on the liver. Mephenytoin, an older seizure medicine, has been linked to liver problems on its own, and concentrated rhubarb root contains compounds (anthraquinones like emodin and rhein) that some animal studies and a handful of human case reports tie to liver injury.
Stacking two things that may stress the liver could, in theory, add up. Watch for warning signs like yellowing skin or eyes, dark urine, belly pain, nausea, or unusual tiredness. Keep in mind this is a theoretical concern, and rhubarb stalks in food are a different story than strong root supplements. Please check with your pharmacist or doctor before combining them.

Clinically significant. Avoid the combination, or use only under close professional supervision.
Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.
Generally not serious. Be aware of it and mention it to your pharmacist.
- LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Mephenytoin page
“Anyone taking mephenytoin (Mesantoin) for seizures should read this page with genuine care rather than as background noise. The biggest cluster of interactions piles sedation on top of a drug that already slows the nervous system, and alcohol, whether beer or wine, sits at the serious end of that group.”

About Rhubarb
What is Rhubarb?
- Constipation
- Digestive upset
- Menopausal symptoms
- Kidney and liver conditions (traditional)
- Pancreatitis (hospital/traditional use)
Rhubarb root has a long history of use as a laxative and in traditional Chinese medicine, and its edible stalks are a common food. Most medicinal claims are backed by limited or low-quality evidence, and the root can cause cramping and diarrhea. The leaves are toxic and should never be eaten, and rhubarb supplements are best avoided in pregnancy and breastfeeding.
