Rifampin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sage interacts with.

Theoretically, sage might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP3A4. So far, this interaction has not been reported in humans.

Here's the gist for rifampin: lab studies hint that sage might slow down a liver enzyme called CYP3A4. Since rifampin is partly broken down by that enzyme, the theory is that sage could let a little more rifampin linger in your body, which might mean more of its usual effects, like stomach upset, harmless orange-red coloring of urine and tears, or strain on the liver.

I want to be honest, though: this is only seen in test-tube research and has never actually been reported in real people. Everyday cooking with sage is very unlikely to matter. Concentrated sage extracts or essential oil are a different story. Before adding a sage supplement, just check with your pharmacist or doctor first.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.

About Rifampin

What Is Rifampin Used For?

Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor

Rifampin treats all forms of tuberculosis. It is always used as part of a combination of TB medicines, not on its own.

It also treats asymptomatic meningococcal carriers. These are people who carry the bacteria Neisseria meningitidis in the nose and throat without being sick. Rifampin is not used to treat an actual meningococcal infection.

Rifampin capsules are taken by mouth. Rifadin IV and Rifampin for Injection are given through a vein for the initial treatment and retreatment of tuberculosis when the medicine cannot be taken by mouth.
