Sofosbuvir falls under P-GLYCOPROTEIN SUBSTRATES — the group Schisandra interacts with.

Schisandra might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that schisandra extracts and constituents such as schisandrin B inhibit P-glycoprotein mediated efflux in intestinal cells and in P-glycoprotein over-expressing cell lines. Additionally, a small clinical study shows that schisandra increases the peak concentration and overall exposure to talinolol, a P-glycoprotein probe substrate. Theoretically, schisandra might inhibit the efflux of other P-glycoprotein substrates.

Here's the gist for Sofosbuvir (Sovaldi). Your body uses a kind of cellular bouncer called P-glycoprotein to push this drug back out of your gut and other tissues. Schisandra appears to block that bouncer, so in theory more sofosbuvir could stay in your system, which might raise its levels and the chance of side effects like tiredness, headache, or nausea.

I'll be honest, though: this is based mostly on lab and animal work, plus one small study using a different drug, so it's more of a caution than a proven problem. Since you're treating hepatitis C and sticking to your regimen really matters, I'd run any Schisandra use past your pharmacist or doctor before adding it.

Human evidence is limited and most claims are not well proven, but it appears reasonably well tolerated by most healthy adults when used short term. About Sofosbuvir

What Is Sofosbuvir Used For?

Brand name: Sovaldi

Sovaldi (sofosbuvir) is used to treat chronic hepatitis C virus (HCV) infection. In adults, it treats genotype 1, 2, 3, or 4 HCV — with or without compensated cirrhosis (mild liver scarring that the liver is still managing). It is always used as part of a combination regimen with other antiviral medicines.

In children 3 years and older, Sovaldi is approved for genotype 2 or 3 HCV, with or without compensated cirrhosis, in combination with ribavirin. Sovaldi is also used with ribavirin in adults with HCV-related liver cancer (hepatocellular carcinoma) who are waiting for a liver transplant, to help prevent HCV from returning after transplant.
