Buproprion, Dextromethorphan falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Shiitake Mushroom interacts with.

Theoretically, shiitake mushroom might decrease levels of drugs metabolized by CYP2D6.
In vitro studies suggest that the shiitake mushroom extract AHCC might induce the CYP2D6 enzyme. This effect has not been reported in humans.

Auvelity is an interesting combo. The dextromethorphan side is broken down by an enzyme called CYP2D6, and bupropion is actually added partly to block that enzyme so the dextromethorphan stays in your system and works on mood. So this medicine already depends on careful CYP2D6 balance.

The concern here is theoretical only. In lab (test tube) studies, a concentrated shiitake extract called AHCC seemed to speed up CYP2D6. If that happened in people, it could clear the dextromethorphan faster and possibly make Auvelity work less well. This has never been shown in humans, and eating shiitake as food is very unlikely to matter. If you want to use a concentrated shiitake supplement, just check with your pharmacist or doctor first.
