Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
Why this applies to Galantamine
Galantamine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Shiitake Mushroom interacts with.
In vitro studies suggest that the shiitake mushroom extract AHCC might induce the CYP2D6 enzyme. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for galantamine (Razadyne). One of the liver enzymes that helps clear galantamine from your body is called CYP2D6. In lab dish studies, a concentrated shiitake extract called AHCC appeared to speed that enzyme up. If that happened in real life, your body might break galantamine down a little faster, which could mean slightly less benefit for memory and thinking.
I want to be honest about how soft this is. It's only a theoretical concern from test-tube work, never actually seen in people, and eating shiitake as a normal food is very unlikely to matter. Concentrated AHCC capsules are a different story. If you're considering one, just run it by your pharmacist or doctor first so they can keep an eye on things.

From our pharmacist’s overview on the Galantamine page
“Galantamine (Razadyne) treats Alzheimer's disease, so it is often a caregiver filling the pill organizer alongside memory supplements and morning juice. That pairing deserves genuine care, because three of the 170 supplement ingredients on file are rated major and two of them are everyday items.”

What is Shiitake Mushroom?
- Immune support
- General wellness and nutrition
- Cholesterol and heart health
- Adjunct in cancer care (lentinan, AHCC)
- Antioxidant support
Shiitake is a popular edible mushroom that is nutritious and safe to eat as food for most people. Talk with your pharmacist or doctor before using concentrated shiitake supplements.
About Galantamine
What Is Galantamine Used For?
Brand names: Reminyl, Razadyne
Galantamine — available as extended-release capsules, tablets, and an oral solution — is used to treat mild to moderate dementia caused by Alzheimer's disease. All supported formulations share this indication.
It does not cure or slow the underlying disease, but it can help support cognitive function in people with early to moderate-stage Alzheimer's. Its benefits may lessen as the disease progresses and more brain cells are lost.
