Dextromethorphan falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Shiitake Mushroom interacts with.

Theoretically, shiitake mushroom might decrease levels of drugs metabolized by CYP2D6.
In vitro studies suggest that the shiitake mushroom extract AHCC might induce the CYP2D6 enzyme. This effect has not been reported in humans.

Here's the gist: your body uses an enzyme called CYP2D6 to break down dextromethorphan, the cough suppressant in products like Benylin and Theraflu strips. Some lab studies hint that a concentrated shiitake extract called AHCC might speed up (induce) that enzyme. If that happened, your body could clear the cough medicine a little faster, which in theory might make it work less well at calming your cough.

Honestly, this is very weak evidence. It comes only from test-tube studies and has never been shown in real people. Eating shiitake as a normal food is not a concern here. If you take strong shiitake or AHCC supplements regularly, just mention it to your pharmacist, but there's no reason to worry or change anything on your own.

Dextromethorphan temporarily relieves cough due to minor throat and bronchial irritation, such as the kind that comes with the common cold or breathing in irritants. It also helps reduce the urge to cough so you — or your child — can get to sleep.

Some products, including CVS Long Acting Cough and Premier Value Tussin Cough oral liquids, are also indicated to help loosen phlegm (mucus) and thin bronchial secretions to help drain the airways.
