Why this applies to Doxepin

Doxepin falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Shiitake Mushroom interacts with.

Theoretically, shiitake mushroom might decrease levels of drugs metabolized by CYP2D6.
In vitro studies suggest that the shiitake mushroom extract AHCC might induce the CYP2D6 enzyme. This effect has not been reported in humans.

Here's the quick version: your body partly relies on an enzyme called CYP2D6 to break down doxepin. Lab studies (done in test tubes, not in people) hint that a concentrated shiitake extract called AHCC might rev that enzyme up a bit. If that happened in real life, your body could clear doxepin a little faster, which might make it work less well, so you may not get the same help falling or staying asleep.

The reassuring part: this is theoretical only and has never been seen in humans. Eating shiitake as food is very unlikely to matter. If you're thinking about a concentrated shiitake or AHCC supplement, just run it by your pharmacist or doctor first so they can keep an eye on how well your doxepin is working.
