Why this applies to Nortriptyline
Nortriptyline falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Shiitake Mushroom interacts with.
In vitro studies suggest that the shiitake mushroom extract AHCC might induce the CYP2D6 enzyme. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body uses an enzyme called CYP2D6 to clear nortriptyline out of your system. Lab studies hint that a concentrated shiitake extract called AHCC might speed that enzyme up. If that happened, your nortriptyline could be cleared a little faster than usual, which in theory might leave less in your body and make it work less well. That could mean your depression symptoms aren't as well controlled.
Now the reassuring part: this is only seen in test-tube studies, never in actual people, so it's very much theoretical. Eating shiitake as food is not a worry. If you're thinking about a strong shiitake or AHCC supplement, just run it by your pharmacist or doctor first so they can keep an eye on how you're feeling.
