Here's something worth clearing up. The interaction in our database is really about immune-suppressing medicines, where shiitake's immune-boosting compounds might work against the drug. But Flupenthixol isn't an immunosuppressant at all. It's an antipsychotic, used for conditions like schizophrenia and sometimes depression, and it works on dopamine in the brain, not on your immune system.

So for Flupenthixol specifically, this listed concern doesn't really fit. I wouldn't expect shiitake to meaningfully change how well your Flupenthixol works or its usual effects (things like drowsiness, restlessness, or stiffness). On top of that, this is only a theoretical, lab-based concern. Eating shiitake as food is generally fine, but before adding a concentrated shiitake supplement, check with your pharmacist or doctor.

Theoretically, shiitake mushroom might decrease levels of drugs metabolized by CYP2D6.
In vitro studies suggest that the shiitake mushroom extract AHCC might induce the CYP2D6 enzyme. This effect has not been reported in humans.

Here's the gist for flupenthixol (Fluanxol), an antipsychotic your body partly clears through a liver enzyme called CYP2D6. The concern with shiitake comes from lab studies suggesting one of its concentrated extracts (AHCC) might speed up that enzyme. If that happened in real life, your body could break flupenthixol down a little faster, which might mean slightly weaker effect and less symptom control.

I want to be honest about how soft this is: it's only been seen in test-tube studies, never in actual people, so I wouldn't lose sleep over it. Eating shiitake as food almost certainly won't matter. If you're thinking about a concentrated shiitake or AHCC supplement, just run it by your pharmacist or doctor first so they can keep an eye on how you're doing.
