Fomoterol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Shiitake Mushroom interacts with.

Theoretically, shiitake mushroom might decrease levels of drugs metabolized by CYP2D6.
In vitro studies suggest that the shiitake mushroom extract AHCC might induce the CYP2D6 enzyme. This effect has not been reported in humans.

Here's the gist for your inhaler: lab studies hint that a concentrated shiitake extract called AHCC might speed up an enzyme (CYP2D6) that helps break down certain medicines. Formoterol is partly handled by that enzyme, so in theory faster breakdown could mean a little less drug hanging around, which might soften how well it keeps your airways open.

Honestly, I wouldn't lose sleep over this one. This is theoretical, based only on test-tube work, and it has never actually been seen in people. Eating shiitake as food is very unlikely to matter. If you take strong shiitake or AHCC supplements and notice more wheezing or shortness of breath, check in with your pharmacist or doctor before changing anything.

What Is Formoterol Oral Inhalation Used For?

Brand name: Foradil

Formoterol fumarate inhalation solution is used for the long-term maintenance treatment of airway narrowing (bronchoconstriction) in adults with COPD, including chronic bronchitis and emphysema. It is taken twice daily — morning and evening — as a regular, ongoing treatment.

Important limits: This medication is not for relieving sudden COPD flare-ups or breathlessness in the moment, and it is not approved to treat asthma. A separate short-acting inhaler should always be on hand for sudden symptoms.
