Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
Why this applies to Tamoxifen Citrate
Tamoxifen Citrate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Shiitake Mushroom interacts with.
In vitro studies suggest that the shiitake mushroom extract AHCC might induce the CYP2D6 enzyme. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with tamoxifen: it's actually a prodrug. Your body uses an enzyme called CYP2D6 to convert it into endoxifen, the active form that does the real work against breast cancer. So this enzyme isn't clearing tamoxifen out, it's switching it on.
Lab studies hint that a concentrated shiitake extract called AHCC might speed up (induce) CYP2D6. In theory, a faster enzyme could shift how much active drug you make, which is why we'd rather not gamble with a medicine this important. That said, this is only test-tube data and hasn't been seen in real people, and eating shiitake as food is a different story than taking strong supplements. Please run any concentrated shiitake product past your oncologist or pharmacist first.

- Immune support
- General wellness and nutrition
- Cholesterol and heart health
- Adjunct in cancer care (lentinan, AHCC)
- Antioxidant support
Shiitake is a popular edible mushroom that is nutritious and safe to eat as food for most people. Some of its extracts (like lentinan and AHCC) have been studied as immune support, mainly alongside cancer treatment, but evidence for most supplement uses is limited. Talk with your pharmacist or doctor before using concentrated shiitake supplements.
About Tamoxifen
What Is Tamoxifen Used For?
Brand names: Nolvadex, Soltamox
Tamoxifen is used to treat metastatic breast cancer (breast cancer that has spread) in both women and men, and as adjuvant (after-surgery) therapy for early-stage breast cancer in women. It also reduces the occurrence of cancer in the opposite breast when used as adjuvant therapy.
In women with ductal carcinoma in situ (DCIS) — a very early, non-invasive form of breast cancer — tamoxifen is used after breast surgery and radiation to reduce the risk of developing invasive breast cancer. It is also approved to reduce the incidence of breast cancer in women who are at high risk but have not yet been diagnosed. Soltamox (oral solution) is specifically indicated for all of these uses in adult patients.
