Here's the gist with shiitake and thalidomide. Concentrated shiitake extracts (think lentinan or AHCC supplements) are studied for revving up the immune system. Thalidomide works partly by tweaking your immune response, so the worry is that an immune-stimulating supplement could pull in a slightly different direction and potentially blunt how well thalidomide does its job. With multiple myeloma especially, you don't want anything quietly working against your treatment.

I want to be straight with you: this is a theoretical concern based on lab studies, not real patient reports, so it's far from certain. Eating shiitake as food is very unlikely to matter. The caution is really about concentrated capsule-style extracts. Before adding any shiitake supplement, please run it by your oncologist or pharmacist first.

About Thalidomide

What Is Thalidomide Used For?

Brand name: Thalomid

Thalomid (thalidomide capsules) is approved for two uses. First, it is used in combination with dexamethasone to treat newly diagnosed multiple myeloma — a cancer of plasma cells in the bone marrow.

Second, Thalomid is used on its own to treat the skin symptoms of moderate to severe erythema nodosum leprosum (ENL), a painful inflammatory reaction that can occur in people with leprosy. It can also be used as ongoing maintenance therapy to prevent ENL from coming back. Note: Thalomid is not approved as the sole treatment for ENL when moderate to severe nerve inflammation (neuritis) is also present.
