Soy and Candesartan Cilexetil, Hydrochlorothiazide

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Soy interacts with whole categories of medication. Candesartan Cilexetil, Hydrochlorothiazide falls into 2 of those categories, so it can interact with Soy in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ANTIHYPERTENSIVE DRUGS

1 of 2

Why this applies to Candesartan Cilexetil, Hydrochlorothiazide

Candesartan Cilexetil, Hydrochlorothiazide falls under ANTIHYPERTENSIVE DRUGS — the group Soy interacts with.

Theoretically soy protein may have additive effects with antihypertensive drugs and increase the risk of hypotension.
Although some contradictory research exists, most clinical evidence suggests that consuming soy protein modestly reduces systolic and diastolic blood pressure in individuals with prehypertension or hypertension.
Severity Moderate Likelihood Possible Evidence A

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Atacand HCT pairs candesartan, a blood pressure medicine, with a water pill (hydrochlorothiazide). Both work to lower your blood pressure. The thing to know about soy is that it can gently nudge blood pressure down on its own. So when you regularly eat a lot of soy protein, it may add to what your medication is already doing.

Most of the time this overlap is small and not a problem. But if your numbers drop too low, you could feel dizzy, lightheaded, tired, or off balance, especially when standing up quickly. Everyday soy foods like tofu, edamame, or soy milk are usually fine. Concentrated soy protein or isoflavone supplements are worth running by your pharmacist or doctor first, particularly if your pressure already runs low.

DIURETIC DRUGS

2 of 2

Candesartan Cilexetil, Hydrochlorothiazide also falls under DIURETIC DRUGS.

Theoretically, soy might have additive effects when used with diuretic drugs.
Animal research suggests that genistein, a soy isoflavone, increases diuresis within 6 hours of subcutaneous administration in rats. The effects seem to be similar to those of furosemide. This effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the gist with soy and your Atacand HCT. This combo already includes hydrochlorothiazide, a water pill, plus candesartan for blood pressure. The concern on file is purely theoretical: in one rat study, a soy compound called genistein increased urine output a bit, somewhat like a diuretic. So the worry is simply additive, meaning soy might nudge fluid loss in the same direction your water pill already pushes.

The honest truth is this has never been shown in people, and everyday soy foods like tofu, edamame, or soy milk are very unlikely to matter. Concentrated isoflavone supplements are a bigger question mark. If you're prone to dizziness, low blood pressure, or dehydration, just mention it to your pharmacist or doctor before adding soy supplements.
