Soy and Hydrocodone, Ibuprofen

Here is the interaction we found between these two, with the evidence behind it.

Minor

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Why this applies to Hydrocodone, Ibuprofen

Hydrocodone, Ibuprofen falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Soy interacts with.

Soy might modestly induce CYP2C9 enzymes. However, this effect does not seem to be clinically significant.
In vitro research suggests that an unhydrolyzed soy extract might induce CYP2C9. However, the significance of this interaction is likely minimal. In healthy females taking a specific extract of soy (Genistein Soy Complex, Source Naturals), blood levels of losartan, a CYP2C9 substrate, were not significantly affected.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Vicoprofen pairs hydrocodone (for pain) with ibuprofen, and it's the ibuprofen part that gets handled by the CYP2C9 enzyme. The thought here is that soy might gently nudge that enzyme to work a little faster, which in theory could clear ibuprofen slightly quicker. If that happened, you might get a touch less anti-inflammatory benefit from the ibuprofen side of the combo.

Honestly, though, I wouldn't lose sleep over this one. The evidence is only lab-based, and a human study with a real soy supplement showed no meaningful change in a similar CYP2C9 drug. Regular soy foods almost certainly won't matter. If you take concentrated soy isoflavone supplements, just mention it to your pharmacist so they can double-check.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).

About Soy

What is Soy?

  • Menopause symptoms (hot flashes)
  • Heart and cholesterol health
  • Bone health
  • Plant-based protein source
  • General nutrition

Soy is a nutritious bean that is a staple food and a popular source of plant protein and isoflavones. Eating soy foods as part of a balanced diet is generally considered safe for most people, and there is moderate evidence it may modestly help cholesterol and some menopause symptoms, though results vary. Talk with your pharmacist or doctor before using concentrated soy isoflavone supplements, especially if you have a history of hormone-sensitive conditions.
