Soy can lower blood glucose and have additive effects with antidiabetes drugs.
Clinical research shows that whole soy diets and soy-based meals reduce fasting glucose levels in diabetic and non-diabetic individuals. Also, individuals following a soy-based meal replacement plan seem to require lower doses of sulfonylureas and metformin to manage blood glucose levels when compared with individuals following a diet plan recommended by the American Diabetes Association.

Here's the simple version: Avandamet is a combo pill that pairs metformin with rosiglitazone to bring your blood sugar down. Soy, especially as a regular part of your meals or a soy-based meal plan, can nudge blood sugar a little lower on its own. Put the two together and the effects can stack up. Good quality research backs this, so it's worth knowing about.

The practical concern is having blood sugar drop too far. Watch for shakiness, sweating, dizziness, a fast heartbeat, or feeling foggy. This usually isn't a reason to avoid soy foods, but if you start eating a lot more soy or switch to soy-based meal replacements, check in with your pharmacist or doctor. Your medication dose may need a small adjustment, and don't change it on your own.

Metformin, Rosiglitazone also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Soy might modestly induce CYP2C9 enzymes. However, this effect does not seem to be clinically significant.
In vitro research suggests that an unhydrolyzed soy extract might induce CYP2C9. However, the significance of this interaction is likely minimal. In healthy females taking a specific extract of soy (Genistein Soy Complex, Source Naturals), blood levels of losartan, a CYP2C9 substrate, were not significantly affected.

Here's the short version: in Avandamet, the rosiglitazone part is partly cleared by an enzyme called CYP2C9, and soy might nudge that enzyme to work a little faster. In theory, that could lower rosiglitazone levels slightly and make it a touch less effective at controlling blood sugar. Metformin isn't really handled by this enzyme, so it stays out of this picture.

The good news is that this concern comes only from lab studies, not from real patients, and a human study with a soy supplement showed no meaningful change in a similar drug. So I wouldn't lose sleep over soy in your diet. If you take concentrated soy isoflavone capsules, just mention it to your pharmacist or doctor so your blood sugar can be watched.
