Intestinal bacteria are responsible in part for converting soy isoflavones into their active forms. Antibiotics may decrease the amount of intestinal bacteria and decrease its ability to convert isoflavones.

Here's the thing to know about this pairing: it's the soy that may take a hit, not your medication. Your gut bacteria help turn soy isoflavones into their more active forms. Methenamine works as a urinary antiseptic by releasing formaldehyde in acidic urine, and while it isn't a typical broad antibiotic, any drug that nudges your gut bacteria could in theory leave fewer of those helpers around to activate soy's compounds. So the worry is that you might get a little less benefit from soy, not more side effects.

This is only a theory based on lab work, so I wouldn't lose sleep over it. Eating soy foods normally is fine. If you take concentrated soy isoflavone supplements for a specific reason, just mention it to your pharmacist.
