Concomitant use of serotonergic drugs with St. John's wort can lead to increased adverse effects and increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstriction disorders.

Here's the thing about Alosetron: it works on serotonin pathways in your gut (it's a 5-HT3 blocker that calms an overactive bowel in IBS-related diarrhea). St. John's wort also nudges up serotonin activity in the body. On paper, combining the two could add together and theoretically raise your risk of too much serotonin, which can cause symptoms like agitation, a racing heart, shakiness, sweating, or in rare cases something more serious called serotonin syndrome.

I want to be honest about the strength of this one. It's theoretical, based on how these two work rather than real-world reports, so it's not a reason to panic. Still, since both touch serotonin, I'd loop in your pharmacist or doctor before pairing them rather than starting on your own.

About Alosetron

What Is Alosetron Used For?

Brand name: Lotronex

Alosetron tablets — available as Alosetron, Alosetron Hydrochloride, and Lotronex — are approved only for women with severe diarrhea-predominant irritable bowel syndrome (IBS) whose symptoms have lasted 6 months or longer and who have not gotten adequate relief from conventional therapy. Other causes of GI symptoms must be ruled out first.

To qualify as severe, diarrhea-predominant IBS must include diarrhea plus at least one of the following: frequent and severe abdominal pain or discomfort, frequent bowel urgency or fecal incontinence (accidental stool leakage), or significant disruption to daily activities. Clinical studies have not confirmed the same benefit in men.
