Here's the gist with Limbitrol, which combines amitriptyline (an antidepressant) and chlordiazepoxide (a benzodiazepine for anxiety). St. John's wort revs up a liver enzyme called CYP3A4 that helps clear both of these out of your body. When that enzyme works faster, your medicine can get broken down too quickly, so the levels in your blood may drop. For you, that can mean your depression or anxiety isn't controlled as well as it should be, and stopping suddenly low can even bring on withdrawal-type feelings.

This isn't a guessing game either, real studies have shown St. John's wort lowering similar medicines. Please don't start or stop anything on your own. Loop in your pharmacist or doctor first so they can watch your symptoms and adjust safely.

Limbitrol DS combines amitriptyline (an antidepressant) with chlordiazepoxide (a calming medicine). The piece that matters here is the amitriptyline. It raises serotonin in your brain, and St. John's wort can nudge serotonin upward too. Put them together and those effects may stack up.

This concern is mostly theoretical, based on how the two work rather than strong studies, so I don't want to alarm you. Still, the realistic worry is too much serotonin, which can show up as agitation, sweating, shaking, a racing heart, or confusion. Amitriptyline can already cause drowsiness and dizziness, and the chlordiazepoxide adds to that sleepiness. Please check with your pharmacist or doctor before pairing these, and don't stop anything on your own.

Here's the thing to keep in mind with Limbitrol DS (which combines amitriptyline and chlordiazepoxide) and St. John's wort. This particular concern isn't about your liver enzymes or drug levels at all. It's about your skin and sunlight. St. John's wort contains a compound called hypericin that can make your skin more sensitive to the sun, and amitriptyline can do the same thing on its own. Stacking them together could, in theory, mean you sunburn more easily or get a rash on sun-exposed areas.

Honestly, this one is fairly mild and mostly theoretical. Research suggests it usually takes very high doses of St. John's wort to trigger real sun sensitivity. Still, simple sunscreen and a hat go a long way. Just loop in your pharmacist or doctor before combining the two.

St. John's wort may increase the metabolism and reduce the levels of CYP2C19 substrates.
Preliminary clinical research in healthy males shows that taking St. John's wort for 14 days induces CYP2C19 and increases metabolism of mephenytoin (Mesantoin). In patients with wild-type 2C19 (2C19*1/*1) metabolism was almost 4-fold greater in subjects who received St. John's wort compared to placebo. In contrast, patients with 2C19*2/*2 and *2/*3 genotypes did not demonstrate a similar increase in metabolism. Theoretically, St. John's wort might increase metabolism of other CYP2C19 substrates.

Limbitrol DS combines two active medicines: amitriptyline (an antidepressant) and chlordiazepoxide (a benzodiazepine for anxiety). Both are active drugs already, not prodrugs, so they don't need to be switched on to work. The concern here is that St. John's wort speeds up some of the liver enzymes (including CYP2C19) that break these medicines down. When your body clears them faster, the levels in your blood can drop, and you may get less relief from your depression or anxiety symptoms.

This is rated a moderate, possible interaction based on lower-quality evidence, so it's not a sure thing, and how much it matters can depend on your own genetics. Still, it's worth being careful. Please don't start, stop, or change anything on your own. Talk with your pharmacist or doctor first so they can watch for any return of symptoms.

Here's the thing with Limbitrol DS, which combines amitriptyline (an antidepressant) and chlordiazepoxide (a benzodiazepine for anxiety). There is contradictory research about the effect of St. John's wort on CYP2C9. Some in vitro research shows that St. John's wort induces CYP2C9, but to a lesser extent than CYP3A4. St. John's wort also induces metabolism of the S-warfarin isomer, which is a CYP2C9 substrate. Other research shows that St. John's wort 300 mg three times daily for 21 days does not significantly affect the pharmacokinetics of a single 400 mg dose of ibuprofen, which is also a CYP2C9 substrate. Until more is known, use St. John's wort cautiously in patients who are taking CYP2C9 substrates.

Limbitrol combines amitriptyline (a tricyclic antidepressant) with chlordiazepoxide (a benzodiazepine). St. John's wort can speed up some of the liver enzymes that clear these medicines, so the worry here is that your body may break the drugs down faster and leave less in your system. In practice that could mean your depression or anxiety symptoms creep back, or the medicine just doesn't seem to work as well as before.

The evidence on this particular enzyme (CYP2C9) is mixed and fairly weak, so this may or may not happen. But there's a bigger concern: St. John's wort also affects serotonin, and combining it with amitriptyline can raise the risk of too much serotonin. Please don't start or stop anything on your own. Talk with your pharmacist or doctor first.
