P-GLYCOPROTEIN SUBSTRATES

Chlortetracycline falls under P-GLYCOPROTEIN SUBSTRATES — the group St. John's Wort interacts with.
St. John's wort induces P-glycoprotein. P-glycoprotein is a carrier mechanism responsible for transporting drugs and other substances across cell membranes. When P-glycoprotein is induced in the gastrointestinal (GI) tract, it can prevent the absorption of some medications. In addition, induction of p-glycoprotein can decrease entry of drugs into the central nervous system (CNS) and decrease access to other sites of action.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the gist for chlortetracycline (Aureomycin), an antibiotic you take to clear up an infection. St. John's wort revs up a transporter in your gut called P-glycoprotein, which acts like a little pump. When that pump speeds up, it can push some of the antibiotic back out before your body absorbs it, leaving less of the drug in your system to do its job.
With an antibiotic, the worry isn't extra side effects, it's the opposite: not enough medicine to fully knock out the infection. That can mean a treatment that doesn't work as well. This is rated a major, likely interaction, though the evidence is only moderate. Please don't start or stop anything on your own. Run it by your pharmacist or doctor first.

St. John's wort might increase photosensitivity reactions due to its hypericin content. However, some clinical research shows that very high doses of St. John's wort are needed to produce phototoxicity in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for this pairing. Chlortetracycline is an antibiotic that can already make your skin extra sensitive to sunlight, meaning you may sunburn faster than usual. St. John's wort carries a compound called hypericin that can do the same thing. Stack the two together and, in theory, you might burn more easily or react more strongly to sun and tanning beds.
I'd keep this in perspective, though. This concern is theoretical, based on how each acts rather than real-world reports, and studies suggest you'd need quite high doses of St. John's wort to cause phototoxicity on its own. Still, it's smart to cover up, use sunscreen, and check with your pharmacist or doctor before combining them, since St. John's wort has other serious interactions too.
