Here's the tricky part with this pairing. Your body uses a liver enzyme called CYP3A4 to process hydrocodone, and St. John's wort speeds that enzyme up. So instead of building up, your hydrocodone gets cleared out faster, which can mean weaker pain control than you expect.

There's a second wrinkle. Hydrocodone is partly turned into a stronger painkiller in your body, so the way levels shift can be unpredictable. If you're relying on a long-acting form for round-the-clock pain, you might notice your pain creeping back. And if you ever stopped the herb, your hydrocodone levels could swing the other way. Please don't change anything on your own. Talk with your pharmacist or doctor first so they can keep your pain controlled safely.
