Here's the wrinkle with Vicoprofen (which pairs hydrocodone with ibuprofen): St. John's wort speeds up a liver enzyme called CYP3A4, which is one of the pathways that handles hydrocodone. Part of what makes hydrocodone work is its conversion into a stronger active form, so revving up this enzyme can actually shift how much pain relief you get and make its effects harder to predict.

The bottom line is that mixing the two can throw off your pain control in ways that are tough to forecast, and it doesn't help the ibuprofen side either. Because this is rated a major, likely interaction, please don't start or stop anything on your own. Talk with your pharmacist or doctor first so they can guide you safely.

Theoretically, St. John's wort might increase the likelihood for photosensitivity reactions when used in combination with photosensitizing drugs.
St. John's wort might increase photosensitivity reactions due to its hypericin content. However, some clinical research shows that very high doses of St. John's wort are needed to produce phototoxicity in humans.

Here's a different kind of interaction than the usual liver-enzyme worries. Both St. John's wort (because of a pigment called hypericin) and the ibuprofen part of Vicoprofen can each make your skin a little more sensitive to sunlight. Put them together and, in theory, you might sunburn more easily or react more to UV light. This isn't about the hydrocodone changing strength, it's just a possible skin and sun issue.

The good news: this is only a theoretical concern based on how these ingredients behave, and research suggests it takes very high doses of St. John's wort to actually trigger phototoxicity. Still, if you take both, be a bit smarter about sun exposure (sunscreen, hat, shade). And since St. John's wort interacts with many medicines, please run it by your pharmacist or doctor first.

St. John's wort may increase the metabolism and reduce the levels of CYP2C9 substrates.
There is contradictory research about the effect of St. John's wort on CYP2C9. Some in vitro research shows that St. John's wort induces CYP2C9, but to a lesser extent than CYP3A4. St. John's wort also induces metabolism of the S-warfarin isomer, which is a CYP2C9 substrate. Other research shows that St. John's wort 300 mg three times daily for 21 days does not significantly affect the pharmacokinetics of a single 400 mg dose of ibuprofen, which is also a CYP2C9 substrate. Until more is known, use St. John's wort cautiously in patients who are taking CYP2C9 substrates.

Vicoprofen pairs two medicines: hydrocodone (an opioid painkiller) and ibuprofen (an anti-inflammatory). The ibuprofen part is partly broken down by an enzyme called CYP2C9. St. John's wort can speed that enzyme up, which in theory could clear ibuprofen a little faster and make the pain-and-swelling relief feel weaker. The good news is that an actual study of St. John's wort with ibuprofen found no meaningful change, so this effect looks small and uncertain.

One thing worth noting: St. John's wort raises serotonin, and the hydrocodone half also nudges serotonin, so combining them carries a low but real risk of feeling jittery, sweaty, or restless. If you take both, please run it past your pharmacist or doctor first.
