hydrocortisone, pyrilamine, chlorpheniramine falls under SEROTONERGIC DRUGS — the group St. John's Wort interacts with.
Concomitant use of serotonergic drugs with St. John's wort can lead to increased adverse effects and increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstriction disorders.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This is a topical product (HC Derma-Pax) that you apply to the skin. It pairs a steroid (hydrocortisone) with two antihistamines (pyrilamine and chlorpheniramine) to calm itching and irritation. The interaction flag here comes from grouping it under "serotonergic drugs," but honestly, that label is a bit of a stretch for a cream you rub on your skin. These antihistamines have only very weak, indirect effects on serotonin, and very little is absorbed through intact skin.
The concern, that St. John's wort plus serotonergic medicines could raise serotonin too much, is theoretical and based on pharmacology, not real cases with a product like this. For a topical lotion, the practical risk is low. Still, it never hurts to mention what you're using to your pharmacist or doctor.
