St. John's wort induces CYP3A4 enzymes and increases metabolism of CYP3A4 substrates. Clinically significant interactions have been reported with St. John's wort products containing hyperforin 1 mg or more.

This combo pill pairs ibuprofen (an anti-inflammatory) with oxycodone (an opioid pain reliever). The oxycodone half is processed by a liver enzyme called CYP3A4, and St. John's wort speeds that enzyme up. The likely result is that your body clears oxycodone faster, so you may get less pain relief than expected.

In practical terms, your pain may not be as well controlled, and you might feel tempted to take more, which raises the risk of stomach upset or bleeding from the ibuprofen side. There's also a real concern: if you stop the herb suddenly, oxycodone levels can climb back up. Because oxycodone is a strong opioid, please loop in your pharmacist or doctor before mixing these.
