St. John's wort might increase photosensitivity reactions due to its hypericin content. However, some clinical research shows that very high doses of St. John's wort are needed to produce phototoxicity in humans.

Here's the gist with CoAprovel (irbesartan plus hydrochlorothiazide) and St. John's wort. The hydrochlorothiazide part is what's called a photosensitizing medicine, meaning it can make your skin react more easily to sunlight, so you may sunburn faster than usual. St. John's wort contains a compound called hypericin that can do something similar. Put them together and, in theory, you could be a bit more sun-sensitive.

I want to be honest about how strong this is: it's mostly a theoretical concern, and research suggests you'd need very high doses of St. John's wort to actually trigger trouble. Still, it's smart to use sunscreen and check with your pharmacist or doctor before combining them, especially since St. John's wort has bigger interactions too.

St. John's wort may increase the metabolism and reduce the levels of CYP2C9 substrates.
There is contradictory research about the effect of St. John's wort on CYP2C9. Some in vitro research shows that St. John's wort induces CYP2C9, but to a lesser extent than CYP3A4. St. John's wort also induces metabolism of the S-warfarin isomer, which is a CYP2C9 substrate. Other research shows that St. John's wort 300 mg three times daily for 21 days does not significantly affect the pharmacokinetics of a single 400 mg dose of ibuprofen, which is also a CYP2C9 substrate. Until more is known, use St. John's wort cautiously in patients who are taking CYP2C9 substrates.

Here's the gist for your CoAprovel, which combines irbesartan (a blood pressure pill) with hydrochlorothiazide (a water pill). The irbesartan part is partly handled by a liver enzyme called CYP2C9. St. John's wort can speed that enzyme up, which in theory means your body could clear irbesartan a little faster, leaving less of it to do its job.

If that happens, the worry is simply that your blood pressure may not be controlled as well as before. Honestly, the evidence here is mixed and on the weaker side, so this may not be a big deal for many people. Still, since blood pressure is something you don't want creeping up unnoticed, please check with your pharmacist or doctor before pairing these, and keep an eye on your home readings.
